Patients recently exposed to or COVID-19 Positive or Patients: If you have tested positive for COVID-19 or have recently been exposed to someone with COVID-19, please contact our office to determine if your appointment needs to be rescheduled.
Paperwork: Registration must be completed prior to your arrival. Due to social distancing and limited space, we do not have designated space to complete the registration forms in our office. Visit this link Patient Registration and click the “Patient Registration” box to complete all of necessary forms and attach images of your valid photo ID and medical/dental insurance cards.
Parking: Street parking or the Pearl parking garage are available for patient parking. There are fees associated with both options. Our office provides validation to reduce the garage parking fee to $9.00.

Payment: We are not accepting cash at this time; however, we are accepting all major credit cards or checks as payment.
